Naso Tang hole in the head

Anyone know what might be causing this? No other fish are effected.

Current Aquarium(s) Description: 120 all glass, Dual 250 MH, 216 watts T5 and a mix of just about anything I can land my hands on

In tangs, it's usually caused by a poor diet (too many meaty foods and not enough veggies). The good news is that it's reversible.

I been accused of having a hole in my head before too. Too much meat, huh? Maybe I should become a vegetarian after all. Oops sorry wrong thread.

I agree. That tang is way to skinny and does not look well fed.
