dead xenia

Ok when I moved my xenia looked completely dead (it's just a small frag) and I decided to put it in the tank anyways. It made a comeback and was looking pretty good for a bit but now it looks dead again. It still has it's color but the tentacles have almost all lost the little fingers and just have white tips?

Any hope left or should I ditch it?

They seem to be really susceptible to ammonia/nitrite/temp changes b/c mine shriveled up was one of the first in the tank to kick the bucket, but as you can see they don't give up easily lol.

Get it back under stable conditions and I'm sure it will make a comeback.
