Problems with Clown

Hi, I have had a Yellowstripe Maroon Clown and carpet anemone for a few years. The clown has done great with the anemone. I just did a water change yesterday and this morning the clown looks pretty beat up - several white spots on his body and a tentacle from the anemone stuck to his tail. It almost looks like he lost his slime coating and can't host in the anemone anymore. Anybody ever heard of this happening before? Anything I can do?
 
Sounds like something has stressed out your clown pretty bad which could cause problems with its slime coat. It could also be brooklynella or ich which would also affect its slime coat.

Please list all your parameters, tank mates and any changes you might have made besides the water change.

A pic would also be nice if possible.

Thanks all - I will report parameters here tomorrow. I will also change the UV bulbs and try to get a pic of him to post. I feed the nem every 3 days or so. Thanks Nick and Emmett.
