Why am I Loosing my Hermits?

I'm not sure you're using the test kits properly every time, 1/2 of the posts you make are about your tests coming out messed up.

I have nitrate problems just like the tests suggested.

I did the phosphate test like 3 times on the tap water and it came out completely 0 but the tank water immediately turned blue.

It may have come out as 0 because those test kits are made to test saltwater, not tap water. You need to test freshly mixed SW to see if it has phos or nitrates.

It has 2 cards. One for saltwater and one for freshwater. Doesn't that mean it works on both??

Maybe, I'm assuming you're not using the API one then.

As you can see in the picture it has two cards, "Saltwater Phosphate Color Card", and "Freshwater Phosphate Color Card".
