They will sometimes pop their head off if under stress. If it's outside it's tube and it's head is popped off, I'd put it into a refugium or other calm area of the tank and just wait.
How do ya know if it's dead? It starts to rot. As long as it's not falling apart or looking slimy - leave it alone.

I agree with the others. If it`s crown does come off it`s because of stress. It will return if stress results are taken care of. My experience has been if it comes out of it`s tube there is not much hope. Others have told me that it will regrow another but I have never seen that.

Mine was plucked out by an Angelfish once. I picked it up off the sand and slid it back into it's tube which was half missing and moved it to another tank. I put it into a LR hole and it survived. Still alive after 2 years!
