Same thing, it needs to be constantly changed/replaced. Atleast every week to keep the detrious from collecting and rotting away.

but does the filter bay where the tank water goes into the weir then the filter bay require any media at all?

I'd say no...Like Biff said with hers all that stuff will settle in your fuge and you can suck it out or add some CUC to your fuge to eat it...sort of the same theory as the algea scrubber...Grow it elsewhere and it wont grow in your tank.

get nitrates down basics:
Lots of rock aquascaped to prevent dead flow areas
good FLOW
do NOT overstock
cheato with STRONG light
Change floss every 2 or 3 days
Use RO water
50% water changes help alot
